Nancy Grace is adamant ... her nip did NOT slip on "Dancing with the Stars" last night ... and insists the object that poked out of her dress last night was merely a PASTY!

Grace tells TMZ ... "When I got dressed, I was wearing Petals (nipple covers) and an industrial strength bra ... my dancing dress also had a bra sewn into it."

Nancy adds, "I have been judged guilty without a trial ... I will go to my grave denying the nip slip."

Rapper Kreayshawn -- the 21-year-old behind that "Gucci Gucci" song -- claims someone hacked her Twitter account and leaked nude pics that she posed for when she was underage.

Kreayshawn went online this morning and explained the situation -- saying, "My twitter got hacked today by some anti-hollywood extremists."

"They sent out wack ass tweets and promoted their odd message of anti-jew stuff and deleted my twitter.. Oh and they leaked out private photos of me while I was underage."

He was Jennifer Lopez' first husband ... her first divorce -- and now, nearly 15 years later, he's a few signatures away from becoming J.Lo's first ex to whip it out for Playgirl magazine.

A rep for the adult rag tells TMZ, it's true ... Ojani Noa and Playgirl are currently hammering out a deal for J.Lo's first ex to appear in the magazine -- but ONLY if he agrees to go full-frontal.

And as long as Playgirl ponies up, that shouldn't be a problem -- Ojani tells us, "If the money is right, especially for full nude, then I would be flattered to pose for Playgirl."

ABC says the network DID have a 5-second delay in place on "Good Morning America" this morning ... but insists Nicki Minaj's nip slip just happened to fall through the cracks.

A spokesperson for the network tells TMZ, "Although we had a five-second delay in place for the Nicki Minaj concert on GMA , the live East Coast feed of the concert regrettably included certain fleeting images of the performer that were taken out of later feeds of the broadcast in other time zones."

The Parents Television Council is blasting "Good Morning America" for airing Nicki Minaj's nip slip on live TV this morning ... saying the whole thing COULD have been prevented with a 5-second delay.

TMZ spoke with Tim Winter, president of the PTC, who tells us, "For the umpteenth time in recent memory a morning news show has included inappropriate content for children and families."

He adds, "Instead of asking for forgiveness, they need to stop apologizing and implement the five second delay that so many Americans have been calling for.”

Nicki Minaj wasn't the only special guest on "Good Morning America" this morning -- her left nipple made an appearance as well!

During her live performance on the show, Nicki's left breast popped out of her VERY low cut top ... revealing her full nipple to the entire East Coast viewing audience.

"GMA" hasn't aired on the West Coast yet ... but we're guessing ABC will most likely edit out the racy shot.

Nothing's signed yet -- and we're told Tareq is adamant about NOT going full-frontal -- but sources in both camps tell us, it's practically a done deal ... aside from a few undersized details.

FYI, the polo-themed picture on the left is no accident -- we're told the plan is to shoot Tareq riding into the sunset on a HORSE ... completely naked.

The "True Blood" star is slated to go full frontal in the HBO miniseries "Mildred Pierce" -- and at a recent NY press event explained, "Let’s just say, I had to wear a wig because it was in the 30s, and everything had to look like it was in the 30s."

According to Wood, her co-star Kate Winslet encouraged her to do it -- “I looked at Kate and she was like, ‘You’ve got to do it. Trust me, it’s so brave. Put a merkin on and you’ll be fine.”

TMZ was approached several days ago by someone claiming to have the sex tape. The person sent us a short video and two photos. We'll keep this PG-13 and just say ... the people in the video are both givers.

The video and photos are clear -- it's Usher and Tameka.

Sources close to Usher -- without specifically admitting the existence of a sex tape -- tell us they believe this video could have been among the things Usher had stolen out of his car back in December 2009. Usher reported more than $1,000,000 worth of jewelry and electronics taken -- including two laptop computers.

A rep for Usher had no comment.

The photos in question were taken by Lyndon Chubbuck ... and scheduled to go on display at an art show later this week at Supperclub Los Angeles.

According to Lyndon, his works of art were nabbed somewhere between the printer and an art show last weekend.

Lyndon says he plans to file a police report tomorrow -- but strictly for insurance purposes because he believes, "The cops aren't really going to put a mad hunt out for my Eva photos."

Nude Eva? Something tells us police might be interested.

43-year-old Braxton went to her Twitter this morning and wrote, "New Year, New opportunities. So I have been considering taking up Playboy's offer to feature me on their cover this year. What you think?"

She noted the spread would be "tastefully done" featuring "Just a lil T and A."

The rep tells TMZ the pictures -- which show Aguilera wearing nothing but pasties and a few strands of fabric -- were "taken in the privacy of Ms. Aguilera's home and were used only in a personal exchange between the star and her stylist."

The rep continues, "We find the actions of the hackers and the press who purchased and/or obtained these photos to be irresponsible and actionable."

After finally admitting she might need to seek financial help to keep a roof over the heads of her 14 kids -- Nadya Suleman told a photog, "I'll do whatever I can ... as long as I'm fully clothed."

Suleman was clearly referring to the $500,000 offer from Vivid Entertaiment -- in which the porn company claimed they'd fork over a check in exchange for one XXX Octo-scene.

And not just artsy "top half" naked -- we're talking full-frontal, birthday suit naked.

Salahi's rep tells us the shoot will go down later this month -- and Michaele is "very excited" about posing down.

We're told Spencer has contacted the Rolls-Royce of porn -- Vivid Entertainment -- and is asking for a meeting.

As we first reported, Heidi has threatened to sue Spencer over a tell-all book -- but she has not made any mention of a sex tape. Unclear if the alleged sex tape with Heidi is pre or post-op.

Vivid honcho Steven Hirsch tells us, "I just got off the phone with Spencer Pratt about a sex tape with Heidi Montag."

He continues, "We are in early negotiations to possibly come to terms for a deal."

Spencer has told a friend that the alleged tape "makes Kim Kardashian look like an amateur."
