BJ Penn slammed his head against a solid wooden bar 3 times in anger ... before the ex-UFC champ went outside and got into a fight this weekend ... bar sources tell TMZ Sports.

We're told ... BJ was a "perfect gentleman" when he arrived to the South Shore Tiki Lounge in Maui on Saturday night -- but became agitated and at one point began banging his head into the bar.

"He was definitely aggravated at something," one source tells us who also says Penn did NOT appear to be drunk.

"He only had one drink. At first he was okay, then he was hitting his head on the bar .... and 20 minutes later he was outside fighting."

As we previously reported, Penn allegedly attacked one of his friends in the parking lot -- and reportedly injured the guy so bad, he was hospitalized for a broken eye socket.

Darren Shahlavi, who appeared in popular TV shows including "Mortal Kombat: Legacy" and "Arrow," died of an apparent overdose ... TMZ has learned.

Shahlavi was found dead January 14 at his L.A. home and law enforcement sources tell us ... it appears he overdosed on prescription meds.

We're told the 42-year-old actor sadly had a history of drug and alcohol abuse.

He pursued a stunt career in Hong Kong in the '90s and eventually came to the U.S. to pursue acting.

In addition to the various shows, Shahlavi starred in the 2012 TV movie "Aladdin and the Death Lamp." He also had a classic fight scene in the cult movie "Ip Man 2."

We're told the L.A. County Coroner is conducting toxicology tests before determining the exact cause of death.

"Arrow" star Stephen Amell -- who fought Darren on the show's first episode -- called him a "great dude & a patient, thoughtful partner."

8:30 AM PT -- The roast was Bieber's idea! We've learned Justin actually called Comedy Central to say he wanted the roast to be a celebration for his 21st birthday ... which is March 1.

Like a tattooed lamb to the slaughter, Justin Bieber has agreed to be the guest of honor at a Comedy Central Roast in March ... and you can already hear Jeff Ross licking his chops.

Ryan Seacrest broke the news on Twitter ... saying, "Yes I got confirmation @justinbieber is getting roasted by @ComedyCentral in march...love u justin but it's time lol #BieberRoast."

Word on the street is that Bieber was game to be roasted for a while ... and had been hoping to work out a deal with the network.

Now, it seems like everything's a go ... and it'll be interesting to see which comics come out to burn Bieber.

Mama June might give Sugar Bear one last chance to work things out -- and she might get PAID to do it ... on a reality TV show.

Sources connected to the now ex-couple tell us they've been contacted by 2 different shows -- one of them being "Marriage Boot Camp" on WE tv -- looking to cast them for an upcoming season.

We broke the story ... June kicked Sugar Bear out of their home last week ... claiming she found out he was two-timing her with several women he'd met online. We're told the marital strife is the real deal -- not created for TV -- but June intends to follow through and meet 'Boot Camp' producers.

She's just not sure Suge will do the same.

Michael Sam clearly wanted to make a statement when he proposed to his longtime BF -- 'cause the football player popped the question right in Vatican City.

Sam posted the pic ... which shows HE was the one who got down on one knee and popped the question to Vito Cammisano last week.

We're told Sam and Vito were inside St. Peter's Basilica at the time.

We're told Beast Mode had originally planned to rock the kicks on Super Sunday (confidence), but when he saw the finished product, he had to have them for the big game this Sunday.

Chris Brown has had his probation revoked in the Rihanna case, in part because he was present at 2 separate nightclubs when people inside were shot, and officials are worried about more shootings and think Chris should serve time behind bars.

Chris showed up for a progress report Thursday, when the judge told him the probation report presented multiple problems. First, the report mentions the shooting last Sunday in San Jose when Chris was on stage. Five people were injured.

The judge also said the probation report expressed concern over another shooting at 1OAK during VMA weekend last August. Again, Chris was on stage and several people were shot, including Suge Knight.

And the judge said Chris left the county for San Jose without getting the court's permission, which is a violation of his probation.

As for why the shootings are an issue, they were raised in the probation report. The judge noted so far there's no evidence Chris was criminally involved, but the probation officer says, "There is also concern that when he performs or attends at some of his public events, people will get shot or seriously injured."

The probation officer also notes Chris has been arrested twice while on probation -- once for assault in DC and also for throwing a rock at his mom's car in rehab. And the officer notes Chris was "making comments about knives and guns during a group [rehab] meeting."

The probation officer -- who thinks Chris is focusing too much on his career rather than community service and rehabilitation -- recommends to the judge that Chris serve time. The officer says the singer has shown "a pattern of making choices that are counterproductive."

The judge will hold a formal hearing March 20. If hizzoner determines Chris "willfully" violated probation, he could be in big trouble. The violations are felonies, which means prison is on the line.

Wrestling legend Kevin Nash is officially off the hook in the Christmas Eve brawl with his son -- after cops determined "Big Sexy" wasn't the one who started the fight.

We broke the story ... both 55-year-old Kevin and his 18-year-old son Tristen were arrested at the family home in Florida after an allegedly drunken Tristen got physical with Kev.

According to the police report, Kevin took Tristen down to the ground hard after Tristen spit in his face. Tristen claimed Kev used his signature choke-slam move and caused him to black out.

But now officials tells TMZ Sports the State Attorney's office investigated the incident and determined "Mr. Kevin Nash was not the primary aggressor in this situation and that criminal charges should not be filed against him."

As for Tristen ... officials say he's not in the clear -- they're still investigating his role in the fight.

The woman claiming Bill Cosby drugged and sexually assaulted her at the Playboy Mansion just over six years ago is filing criminal charges.

Model Chloe Goins and attorney Spencer Kuvin are heading to LAPD headquarters right now ... claiming she's the victim of sexual assault at the hands of Cosby.

As TMZ reported ... Goins alleges in 2008 she woke up naked in a bedroom with Cosby sucking her toes and masturbating after he slipped something in her drink.

Here's what's unclear ... the alleged incident occurred more than 6 years ago, and in California the statute of limitations for every sexual assault except rape is 6 years. We don't know if she's alleging rape.

There was an elephant in the room at a meeting of gay L.A. Republicans ... and the elephant was Justin Bieber

The L.A. chapter of the Log Cabin Republicans had a meeting Monday night at State Social House, one of Bieber's favorite restaurants.

Here's the thing. Justin showed up with 4 friends and wanted his usual perch ... in a placed called The Attic. Problem was ... The gay Republicans were still meetin'.

Justin and his crew were undaunted and just walked into The Attic, sat in the back and ordered chicken wings as the group discussed gay rights in the Republican ranks.

One of the LCR members called in to the Jillian Barberie and John Phillips' show on 790 KABC, and said he thought Justin was just a pretty lesbian ... or a "butcher version of Miley Cyrus.'

Taryn Manning's alleged stalker violated a restraining order by hitting her with a barrage of phone calls and texts ... and a massive "manifesto" about Taryn she uploaded to Reddit ... this according to the criminal complaint obtained by TMZ.

According to the New York District Attorney's Office, Jeanine Heller has been using various phone numbers to text and call the "Orange Is the New Black" star over the last few months, sometimes pretending to be other people.

Some of the messages included:

-- "Why have you tormented me and spread lies ... You said if I died you would celebrate. Well go celebrate. Karma will come."
-- “I miss you. Don’t ruin your pretty smile with cigarettes. This is your fairy godmother."
-- "I was in the hospital last month. I tried to kill myself. I don’t know why you teamed up to hurt me."

The criminal complaint against Heller also makes mention of Heller uploading 385 files onto Reddit in December, including "a 13-page, single-spaced, self-described manifesto claiming to outline the history of the relationship between [Manning] and [Heller}."

Chris brown is finished taking chances ... he's ordered his manager to immediately cancel upcoming shows at any nightclub he thinks is "too hood" after the recent shooting in San Jose.

A source close to Chris tells TMZ the singer called an emergency meeting instructing his crew to pull him out of all future bookings at venues with a history of violence or security issues.

As TMZ previously reported ... five people were wounded when someone fired multiple shots at his show Sunday morning.

We're told Brown doesn't want to be associated with shootings -- and is making the change to protect his image, his ass, and his fans. Not necessarily in that order.

To that point ... Chris performed in a San Diego club the night after the shooting, and declared on Instagram, "No more n***a parties! Only this type s**t!"

Eminem isn't always the angry rapper who rips people on tracks ... sometimes he's the sweetheart who visits the home of a terminally ill fan ... and doesn't tell a soul about it.

The fan in question is 17-year-old Gage Garmo ... who has been diagnosed with Osteosarcoma, a rare form of bone cancer, and has been told he only has one week to live. Just three days ago, Gage's friends and family started up the hashtag #GetGageGarmoToMeetEminem ... in an effort to ... well, it's obvious.

And it worked. We're told Em arranged for a home visit at Gage's place in Michigan ... which is only about 20 minutes from where Em lives.

Miley Cyrus has been burglarized for the THIRD time ... and this time the thief made off with a bundle.

We're told the singer's Toluca Lake home was hit December 16 and the thief made off with a large amount of both her and her brother's personal property. One law enforcement source says the thief got Miley's bank card.

We're told the thief broke into the home. We don't know how.

But cops think they got their man. Rusty Edward Sellner was charged with felony first-degree residential burglary, receiving stolen property and grand theft. He pled not guilty Wednesday.

This is the third time Miley has been hit in a little more than a year. Last June two thieves broke into Miley's home and got her Maserati, jewelry and other items. They were both arrested and convicted.
