1:39 PM PT -- British singer Leona Lewis has now accused Costello of humiliating her at a 2014 charity event ... claiming Costello refused to alter a dress to fit her for a fashion show.

Lewis claimed she was relegated to sit in the audience after Costello, whom she says was assigned as her designer, didn't show up to a fitting the night before the show and offered no explanation. The singer says she doesn't discount Costello's experience but is speaking up because "the pot calling the kettle black in this situation doesn't sit right with me."

Costello tells TMZ ... he's confused because he says he's a huge fan of hers and her team has continuously reached out for him to dress Lewis, even as recently as May 17, 2021.

Michael Costello says he's not out for revenge after claiming Chrissy Teigen's trolling nearly drove him to suicide, but he does want her to contact him if she's genuinely interested in expressing remorse.

The "Project Runway" star tells TMZ ... he's still waiting for Chrissy -- and anyone else who bullied him -- to reach out to him. He says if she truly acknowledges her past actions were wrong, he'd welcome her with open arms for a real conversation.

He said, "I want to be clear -- I do not wish ill on anyone. We are all works in progress and we deserve the opportunity to prove that we can do better. But progress takes time. We must show through actions that we have changed. After all, action speaks much louder than a 10-minute apology written on a notepad."

As we reported ... Michael accused Chrissy of a yearslong campaign to get him blacklisted from the industry after she mistakenly thought he posted something racist back in 2014.

Michael says that post turned out to be doctored, and though he tried his damnedest to set the record straight with Chrissy -- he claims she instead used her power to get brands and companies to drop him.

Michael says he suffered so much from all of this, he thought about taking his own life. He adds, "Nothing takes away from the years of trauma being bullied and blacklisted in the industry has left me. I am not out for revenge. I only wish to only speak my truth because I have been silenced for so long."

Boxing superstar Teofimo Lopez -- one of the best fighters in the world -- has tested positive for COVID, forcing promoters to postpone this weekend's championship fight.

The undefeated 23-year-old -- the unified lightweight world champ -- was supposed to take on George Kambosos in Miami on June 19.

But, Lopez tested positive for coronavirus on Tuesday morning -- and to make things worse, he's symptomatic.

As a result, the fight -- along with the entire card -- is being pushed to August 14 to allow Lopez to recover.

The event -- part of the Triller Fight Club series -- was set to feature some sick musical performances during the event ... headlined by Meek Mill.

Myke Towers and Reggaeton star Lunay were also booked to perform as part of the Triller Live Concert Series.

As for the entertainment portion of the evening, no word on that -- we're working on details.

Teofimo Lopez is 16-0 -- and coming off an incredible victory over Vasiliy Lomachenko, who (at the time) was considered by many to be the best pound-for-pound fighter on the planet.

28-year-old George Kambosos Jr. is also undefeated (19-0) -- and believes a victory over Lopez would rocket him into boxing superstardom.

He's not wrong!

Tiger Woods Returns to L.A. After Car Crash Moving Pretty Well on Crutches!!!

Tiger Woods is back in Los Angeles just 4 months after that horrific crash -- and for a guy who almost lost his leg, he seems to be moving around pretty well!

The golf legend's private jet touched down at an L.A. airport on Monday -- he was joined by his girlfriend, Erica Herman.

Once they deplaned, 45-year-old Tiger -- sporting a black compression sleeve on his right leg -- was moving around on his own, using a pair of crutches.

You can see in the video ... there are moments where Tiger is putting some pressure on the injured leg, though he's still moving with a noticeable limp, as expected.

No word on why Tiger is in town -- it's possible he's here for follow-up treatments with some of the doctors who initially worked on the leg, but we just don't know.

As we previously reported, Woods was very badly hurt during the Feb. 23 crash -- suffering comminuted open fractures in his right leg, which means the bone was shattered in at least 2 places and broke through the skin.

The crash report described the injuries in detail -- "knocked unconscious, laceration to the lower front jaw, bruised right and left rib cage, fractured right tibia and fibula, possible right ankle injury."

A team of surgeons used a rod, screws and pins to stabilize the injuries -- and after weeks of treatment, he was discharged and allowed to go home in mid-March.

He's been recovering at his home in Florida ever since -- but now he clearly feels well enough to travel.

Woods told Golf Digest in May the rehab process is "more painful than anything I have ever experienced."

Still unclear if Tiger will ever play golf again -- either professionally or recreationally -- but the fact he's moving around seems like a good sign.

That's UFC fighter Jamahal Hill telling TMZ Sports the nasty injury he suffered during UFC 263 on Saturday is gonna be FINE ... and he's already thinking about his next fight!!!

The backstory ... Hill was fighting Paul Craig when the 30-year-old got caught in an armbar and Paul pulled Jamahal's left arm in a way it's not supposed to bend!

The good news ... Hill confirms the arm was NOT broken -- "just dislocated pretty badly."

"They were able to pop it back in and the doctor said it’s rock solid. It’s doing all the movement that it needs to, it’s just going to be sore for a while."

So, what's next?

"I’ll follow up with my doctor when I land back at home and we’re going to go from there. Hopefully I can be back in real soon."

As for the pain, Hill says there was a "little discomfort" -- but that's it!

He was more upset about getting caught in the armbar in the first place and losing the fight.

But, Hill's a tough guy and vows to return to the Octagon ASAP -- and he even wants to run it back with Craig!!

"I gotta handle whatever I gotta handle through the loss or whatever, that I just went through. But, definitely I hope he keeps winning, so that way next time we meet is for some serious marbles. But yeah, that’s definitely one I want to run back."

There's more ... Hill also talks about his future and addresses the criticism of the referee who was working the fight.

But, Hill also delivered this message to his fans ...

"I just want everybody know I’m good, I’m alright. I’ll be back."

Michael Costello says he nearly took his own life because he could no longer deal with the trauma -- personally and professionally -- which he claims Chrissy Teigen brought down on him.

The "Project Runway" star's accusing Chrissy of a yearslong campaign to get him blacklisted from the industry, because he says she mistakenly thought he posted something racist all the way back in 2014.

Costello claims no matter what he did to try to convince Chrissy the racism allegations were untrue -- he says Instagram proved it was a Photoshopped comment he didn't write -- she persisted, and told him his career was over.

He also posted screenshots of the alleged messages from Teigen, in which she says, "racist people like you deserve to suffer and die" ... and issues the veiled threat about his career.

Michael claims Chrissy, along with stylist Monica Rose, followed through with this by using their power to get brands and companies to not work with him.

The designer says he's suffered tremendously, and is opening up about it now in an attempt to set himself free from depression and suicidal thoughts.

Costello claims it's gotten so bad recently, he told loved ones he thought taking his own life was the best way out.

Of course, Michael's allegations coincide with Chrissy issuing a lengthy apology for her past behavior online ... which she called trolling, and attributed to her own insecurities and immaturity.

If true, it seems the Michael situation is a different beast with different context ... but it's certainly not any more tolerable.

That's Glen "Big Baby" Davis doubling down on his attack on Kyrie Irving -- saying Kyrie's ankle injury is "karma" for stepping on the Celtics during the playoffs.

Actually, what Glen specifically said was ... "Karma is a motherf**ker!"

Here's the backstory ... Kyrie made a point to step on Lucky the Leprechaun after the Brooklyn Nets beat Boston in Game 4 of their playoff series.

Kyrie famously played for the Celtics from 2017 to 2019 -- but clearly doesn't feel love for the city.

Big Baby, on the other hand, won a championship with the Celtics in 2008 and LOOOOOVESSSS all things Boston.

So, when Kyrie suffered a sprained ankle Sunday while playing the Milwaukee Bucks, Davis sounded off on social media ... saying the whole thing was "karma."

Davis got some pushback on social media -- but he's not fazed, and made a VIDEO going scorched-earth on Kyrie for the disrespect toward his beloved Celtics.

"Everybody mad at me 'cause Kyrie Irving stepped on Lucky and Lucky got his get back," Davis said ... "Don't get mad at me because Lucky got his get back!"

Tigers' Beau Burrows Vomits All Over Mound Sent Down To Minors After

Have a bad weekend? There's probably no way it was worse than Beau Burrows' ...

The Detroit Tigers pitcher got so sick on the mound Saturday -- he blew chunks ALL OVER IT -- and then he was promptly sent down to the minors afterward.

The gross scene all went down in the Tigers' game against the White Sox ... when Burrows entered the contest in the 4th inning with Detroit losing 8-2.

The 24-year-old rookie cruised in his first inning on the bump ... but ran into some major issues when he came back out for the fifth inning.

He walked a batter, gave up two straight hits, then walked another batter ... and even though he went on to strike out the next two guys -- something went terribly wrong.

Check out the footage, as Burrows was trying to close out the inning ... he got visibly ill -- and threw up everywhere ... TWICE.

Burrows tried to stay in the game ... but he was pulled after trainers and coaches came out to talk with him.

But, the worst part? The Tigers sent him down to Triple-A after the game ... meaning he's going to have to work his ass off to get back up to The Show anytime soon.

As for his health, Tigers manager A.J. Hinch didn't seem concerned this was a long-term or serious issue ... explaining his pitcher was simply just nauseous.

CNN's Christiane Amanpour just told viewers she has ovarian cancer -- something she says she'll likely be able to survive ... all due to early detection.

The network's chief international affairs anchor broke the news Monday, announcing she'd recently been diagnosed ... adding that the reason she's been away from work lately is because she had "successful major surgery" to nip it in the bud.

Christiane says she's confident about the outcome, even as she begins several months of chemotherapy to deal with the cancer doctors weren't able to surgically remove.

As for her optimism -- CA attributes that to the fact that she found the cancer early via medical tests/screenings. Christiane made a plea to the women at home who might be watching ... urging them to listen to their bodies, and to get checked out early and often -- because detecting this disease as soon as possible is really half the battle.

The iconic journalist also encouraged women not to let doctors dismiss or diminish their concerns -- which seems to be getting at an important larger issue.

Here's to a speedy recovery, Ms. Amanpour.

Soccer's Christian Eriksen Breaks Silence After Medical Emergency 'I Won't Give Up!'

Just days after he collapsed on the soccer field, Christian Eriksen is speaking out -- vowing to fight in his recovery while doctors continue to hunt for the cause of the medical emergency.

"'Thank you, I won't give up," Eriksen said in a statement issued through his agent to the Italian media outlet, Gazzetta dello Sport.

"I feel better now -- but I want to understand what's happened."

The statement continued, "I want to say thank you all for what you did for me."

As we previously reported, the 29-year-old went into cardiac arrest during Saturday's Denmark vs. Finland game in Copenhagen.

Emergency medial personnel raced onto the field to treat him -- and he was eventually resuscitated with a defibrillator.

Eriksen was transported to a nearby hospital where he continues his recovery. He's currently in stable condition.

The Denmark team doctor, Morten Boesen, spoke with the media on Sunday and addressed the seriousness of Eriksen's situation in the seconds after the soccer star collapsed.

"He was gone," Boesen said ... "And we did cardiac resuscitation. And it was cardiac arrest."

He added, "How close were we? I don’t know. We got him back after one defib. That’s quite fast."

As for what caused the incident, Boesen said specialists are still trying to figure that out.

"I’m not [a] cardiologist, so the details about why it happened and further, I will leave to the experts."

Bottom line ... Eriksen seems to be on the road to recovery, which is a miracle considering how scary things looked on Saturday.

11:33 AM PT -- 6/13 -- The Danish team doctor confirmed that Eriksen suffered a cardiac arrest during his emergency on the field. He's stable now and seems to be doing well, but it's unclear if or when he'll return to playing condition.

10:34 AM PT -- UEFA put out a statement, "Following the medical emergency involving Denmark’s player Christian Eriksen, a crisis meeting has taken place with both teams and match officials and further information will be communicated at 19:45 CET. The player has been transferred to the hospital and has been stabilised."

10:29 AM PT -- According to reports, Erikson was seen raising his hand and appeared conscious as he was being taken off the field -- which seems to suggest he might be okay. Thank goodness. 👏🏽

Christian Eriksen -- a Danish soccer player, who's one of the stars on the national team -- just had a frightening emergency during the first round of the UEFA European Championship ... collapsing on the field and requiring immediate medical attention.

The 29-year-old midfielder fell to the ground Saturday during Denmark's Euro 2020 match against Finland, toward the end of the first half. He was jogging back and about to receive a ball being thrown to him from one of his teammates, when all of a sudden ... Eriksen toppled over without provocation -- not being touched by another player or anything.

Everyone around him instantly recognized he was suffering an emergency and called for the ref to stop the game -- while also waving on team doctors and other first-responders on hand. Footage was captured from the terrifying moment ... and it's clear he'd lost consciousness.

Reports say CPR was rendered to Eriksen on the spot before he was carried off a stretcher for further treatment. His team -- including players from the other side -- were notably concerned, with a majority of them taking a knee out of respect ... a tradition in soccer.

His teammates also linked arms around Ericksen and formed a wall of sorts -- seemingly to give him privacy while he was being tended to. No word yet on his status or what might've caused the collapse. Of course, everyone is sending their thoughts and prayers.

Kelsey Salmon wants the lash tech whose dog bit off her eyelid to take responsibility, starting with covering a mountain of medical bills -- but instead, she says the woman's ducking her.

Kelsey tells us the lash tech hasn't reached out to her even once since the woman's chihuahua jumped up and bit off Kelsey's eyelid, complete with her new lashes.

She says the tech's already blocked her on Instagram, but the 23-year-old from Atlanta wants to make it clear ... she's not trying to get rich with a lawsuit. Kelsey says she just wants her medical bills paid. Not to say that will be cheap -- Kelsey estimates the total could be $20k-$30k, based on the 2 ambulance trips she's already taken and the reattachment surgery.

What she doesn't want is her family getting stuck with the bill, because she says a previous medical drama -- involving blood clots, a stroke and open-heart surgery -- ended up costing them $4 million. She says that's the only reason her family might sue.

The other issue at hand is the dog -- we asked if she thought it should be put down, and you can tell she's really on the fence. Kelsey says she's a dog lover, but also knows she doesn't want anyone else to endure the hell she is right now.

As far as she knows the chihuahua's currently quarantined, and it's up in the air what will happen next.

'Ultimate Slip 'N Slide' Cut!!! That's Not Water ... Diarrhea Outbreak Shuts Down Show

NBC's competition show "Ultimate Slip 'N Slide" almost had to change the title to "Run 'N Don't Shart" ... after multiple crew members came down with a serious case of diarrhea.

So get this ... the workers reportedly came down with bubble guts on the heels of at least one person on set testing positive earlier this month for giardia -- an intestinal infection caused by a parasite.

The crew was filming in the San Fernando Valley before production was shut down indefinitely. According to The Wrap, things were so bad, some crew members collapsed in pain, while others raced into port-o-potties.

A source close to production confirmed to TMZ ... a number of people reported gastrointestinal symptoms and upset stomachs. We're told it's unclear exactly how many, but it was enough to halt production.

Our sources say the studio paused production on June 2 when it was made aware of the giardia case, and, in coordination with various health departments ... production worked with an environmental lab to test the water on location.

We're told the pond, slide pool, water truck and restroom sinks all tested negative, but a positive test came back on Thursday. They had been shooting for 7 weeks with one week left to go. Btw, we're told the hosts, Bobby Moynihan and Ron Funches, are fine.

A rep for Universal Television Alternative Studios, the production company behind the NBC show, tells TMZ ... the health and safety of everyone on its set is its number one priority, so out of an abundance of caution it has made the decision to stop production at the current location. They're trying to figure out where and when to complete the shoot.

Ultramarathon Tragedy Chinese Official Apparently Takes Own Life Amid Probe Into Death Of 21 Runners

An official in China has died -- seemingly taking his own life -- amid an investigation into an ultramarathon tragedy last month that killed 21 elite runners.

The Chinese state-controlled media confirmed Li Zuobi died on Wednesday -- after leaping from his apartment building.

Zuobi was the Jingtai County Communist Party Secretary -- and oversaw a team that was involved with the planning of the 62-mile Yellow River Stone Forest Trail Race which took place on May 22 in the province of Gansu.

As we previously reported, 172 elite athletes participated in the event -- but tragedy struck when a freezing cold storm suddenly moved in just a few miles into the race, punishing the runners with a barrage of hailstones, rain and icy winds.

A 1,200-person search party was launched -- but 21 runners ultimately died due to the severe weather conditions. Several more were hospitalized.

The Chinese government launched a formal investigation into the incident -- and ultimately punished 27 officials for major failures in the race planning.

Among the issues, officials determined the emergency response protocols in place to tackle to serious weather issues were unsatisfactory.

Li was not held personally responsible for the issues -- but his deputy, Zhang Wenling, was fired ... along with other lower-level officials.

As for Li's death, CCTV (the Chinese state-controlled broadcaster) says investigators do NOT believe he was killed ... and have ruled out homicide.

The families of the 21 runners who lost their lives were offered $150k in compensation following the tragedy ... but some have reportedly declined the offer.

One of the runners who perished in the race is 31-year-old Liang Jing -- one of the top ultra-marathon runners in the world. He's survived by his wife and newborn baby.

Dog Bites Woman Eyelid Ripped Off by Chihuahua ... Horrific Injury Photos!!!

Kelsey Salmon's trip to get her eyelashes done ended in the worst possible way ... with a little dog attacking her and biting off the finished product, along with her entire left eyelid!!!

The photos are brutal -- that's a warning for the queasy -- but not as brutal as the experience the 23-year-old from Atlanta has been through since going to a lab technician she found on Instagram.

According to Kelsey ... her normal lash lady was booked, so she went to a lash tech who worked out of her home ... and had a pet chihuahua.

Kelsey says the dog jumped up on her lash bed while she was getting her lashes done, she was petting it ... and everything was fine. But, when she was done, she says she sat up and the dog was on the floor looking up at her.

"Then, out of nowhere -- it happened so fast that I didn't have time to pull back -- he jumped up, bit me in the face and broke my eyelid off."

What Kelsey describes next is a horrific scene of blood and chaos as the ambulance was called, and she was rushed to the ER. After hours of waiting, she says she had to go to another hospital for surgery ... and miraculously, they were able to successfully reattach it.

To make this all worse, Kelsey was getting her lashes done for a birthday trip to Hawaii when this all went down. Fortunately, she still got to go, but with her left eye completely sewn shut ... and she says the doctors told her it could take up to a year to fully heal.

Kelsey's still looking on the bright side, through her one good eye -- "I'm so lucky he didn't grab onto my actual eye because that could have been a thousand times worse."

Mama June Claims Dentist Lied Through His Teeth ... Dentist Responds

6:54 AM PT -- 6/14 The Bev Hills dentist didn't directly respond to Mama June's claims but he tells TMZ ... he has a duty to follow safety protocols, observe professional standards, and follow the rules to ensure the health and safety of its patients.

The dentist pointed to his ethical duty to protect the patient's confidentiality and their privacy ... but he wished Mama June the best and continued success.

Mama June's going after a dentist she claims promised to fix her teeth in exchange for some Instagram publicity, but allegedly pulled a bait-and-switch ... leaving her mouth in major pain.

The reality star's legal team has fired off a letter to a dentist in Beverly Hills. In the document, obtained by TMZ, June says she made a deal with the dentist to fly into town to get her chompers fixed ... and all she'd have to do for payment is tag him and promote his work on social media.

June's rep, Gina Rodriguez, tells us June arrived on Monday for her appointment and booked accommodations for 12 days per the dentist's suggested timeline for the procedure to fix her top and bottom teeth.

According to the letter, the Doc removed her temporary dental implants from a previous procedure but then, "after placing her in a vulnerable (and painful) position," told her he would not complete the process unless she agreed to do media appearances to endorse his services.

June's team says this was never discussed at any point before the removal of her temporary implants, and now she's scrambling to find a new dentist to complete the work because her mouth is exposed ... and she's in a great deal of pain.

June's attorney, Jeffrey R. Klein, is demanding the dentist either complete the work as agreed upon -- or pay her $35k to cover the costs of her travel and getting the work done by another dentist. If he does neither, they say their next move could be a lawsuit.

Tiger Woods Declines TV Role At U.S. Open ... 'He Didn't Want To Do It'

Tiger Woods isn't ready to golf yet ... and apparently, he ain't ready to talk about the sport on TV yet either.

The sport's biggest star -- who's still recovering from a gnarly February car crash -- declined an offer from NBC Sports to help break down next week's U.S. Open at Torrey Pines in CA, according to broadcaster Dan Hicks.

"We were all thinking how good that would be, who better, if he couldn't be there to play it, to voice it and have him a part of the show, but we were rebuffed," Hicks told media members Wednesday ahead of the event's June 17 start date.

"He didn't want to do it, and I totally understand his situation. There is a lot going on in his world right now and there's also a part of Tiger that doesn't want to become this, I don't want to, for lack of a better word, a sideshow at an event where we should be concentrating on what's happening."

Hicks added, "I understand that what’s going on in his world that he wanted to kind of keep it low-key and stay out of the limelight for this one and just hopefully he’ll enjoy it at home watching it on TV."

Tiger -- who won the U.S. Open the last time Torrey Pines hosted the event back in 2008 -- has avoided the spotlight ever since he crashed his ride in Los Angeles County on Feb. 23.

He's hardly spoken about the incident -- revealing only that rehab has been "more painful than anything I have ever experienced" -- and he's only posted one picture of himself since the crash.

As we previously reported, Tiger shattered multiple bones in his leg and required several complicated surgeries and a metal stabilizing rod to fix the issues.

It remains unclear if Woods will ever play competitive golf again.
