Boxing champ Jermain Taylor has been ordered to undergo a mental evaluation ... after a string of arrests and strange behavior that clearly raised some red flags with an Arkansas judge.

Taylor's own attorneys had asked the judge to move the 36-year-old to a state hospital for a full mental evaluation ... after he turned himself in for allegedly firing a gun during an altercation with a family at a Martin Luther King Day parade.

As we previously reported, Taylor kinda sorta acknowledged he fired the gun in a bizarre YouTube video ... in which he blamed his outburst on the fact that several kids at the parade did not have candy.

Taylor has been charged with aggravated assault and endangering the welfare of a minor for the incident. Prosecutors believe Taylor is a danger to himself and others.

The results of Taylor's mental evaluation are scheduled to be presented to the court in April.

"Everybody is saying this isn't the Jermain Taylor they know," Taylor's attorney told ArkansasOnline.com ... "We're trying to figure out who in the heck it is."

Melissa Rivers has just filed a huge lawsuit against the clinic where Joan Rivers' heart stopped beating ... and Melissa's lawyer says the doctors acted like a bunch of groupies and not medical professionals.

The lawsuit -- obtained by TMZ -- claims gross medical malpractice, alleging during the 20 minutes doctors tried to bring Joan back to life, they didn't think to perform a tracheotomy, which facilitates breathing.

Melissa says, "The level of medical mismanagement, incompetency, disrespect and outrageous behavior is shocking and frankly, almost incomprehensible," adding, "Not only did my mother deserve better, every patient deserves better."

In addition to suing the clinic, Melissa has sued Joan's personal doctor, Gwen Korovin. According to the lawsuit, she should not have been permitted in the room where the procedure was being performed, yet she jumped in and announced, "I'll go first."

The suit claims another doctor took cellphone pics of Rivers before the emergency, while she was under sedation, with Korovin in the photos, saying Joan "will like to see these in the recovery area."

According to the lawsuit, Korovin left the room in the middle of the emergency and the allegation is that she left because she knew she wasn't supposed to be there in the first place and "wanted to avoid getting caught." The suit says Korovin simply abandoned Joan.

And get this. According to the suit, in the middle of the emergency, another doctor took issue with a colleague who wanted to scope the affected area, saying, "Renuka, you're such a curious cat. You always want to know what's going on." The suit says the doctor refused to authorize the scope.

Melissa's lawyer spoke to The NY Daily News, which broke the story, and quotes lawyer Jeffrey Bloom who says, "Had doctors acted as physicians for Joan Rivers instead of groupies, Joan Rivers would have been doing 'Fashion Police' last week."

Yorkville Endoscopy has been under intense scrutiny and they are set to lose their accreditation at the end of the month.

Richard Simmons had visitors Friday night ... cops who were doing a welfare check because he's been MIA for more than year ... and guess what? He's fine.

Law enforcement sources tell TMZ ... 2 LAPD officers showed up to Simmons' Hollywood Hills home at 7 PM. Richard answered the door and showed them in. The cops' feedback -- he couldn't have been nicer and more gracious. They say he was articulate, lucid, relaxed and appreciative of their concern.

Law enforcement sources tell TMZ ... the visit was triggered by a long-time friend of Simmons who sent an email to the L.A. County D.A.'s Elder Abuse Unit last month and said he believed the 66-year-old fitness guru was being abused.

TMZ broke the story, Simmons went radio silent a year ago. He hasn't spoken to his friends, gone to his fitness studio or been seen in public. His friends say there is no way Richard would shut himself off this way. His manager says Simmons is simply depressed over a knee injury.

But we've learned the 2 cops who spent an hour with Richard say he seemed genuinely happy. Simmons, who now has a beard, told them he was "exhausted" after spending 30 years in the public eye. He said, "I just want to spend time with myself," although he says he's in regular touch with the people in his life who matter to him. He was alone in the house Friday when cops were there.

Simmons was walking with a slight limp, which is consistent with the knee injury. As for why he hasn't been to his fitness studio for the better part of last year, he said, "I've got people who run things. I don't need to be there." Until he disappeared he was a fixture at the studio.

Pro skier Henrik Harlaut was knocked out cold Saturday during an X Games Aspen event ... and the footage is frightening.

It all went down during the Men's Slopestyle finals ... when Harlaut landed a trick the wrong way and took a gnarly tumble -- leaving the 23 year-old motionless as medics rushed in to help.

The Swedish skier eventually sat up before being stretchered off the mountain ... and waved to everyone letting them know he's OK while plastering a giant smile across his face.

Casper Smart's dog escaped from Jennifer Lopez's house and bit one of her neighbors ... leaving the woman with a broken hand.

Law enforcement sources tell TMZ ... Smart's boxer, named Bear was at JLo's home outside Los Angeles back in December ... when he got loose and clamped down on the neighbor's hand and arm.

Since the attack, Bear has been quarantined at Casper's nearby Woodland Hills home. Authorities are still trying to determine if the dog poses a danger to others.

We're told both of Casper's boxers still spend a lot of time at JLo's house ... since her kids love the dogs. It's unclear if JLo or Casper were at the house when the attack happened, but Animal Control gave Casper a citation for an unleashed dog ... since he's the owner.

Whoopi Goldberg's got back ... problems and it's creating a rift between her, the other "View" co-hosts and producers of the show -- who think Whoopi's new chair is a major eyesore.

When Whoopi returned from her two-week injury sabbatical Tuesday, she brought a new ergonomic chair for support. All the women used the same chair ... so Whoopi's wouldn't look different.

According to our sources, producers felt the chairs looked clunky on camera ... so they yanked them for Wednesday's episode -- but Whoopi stood (or sat) her ground in the new chair.

And yes ... it stood out.

We're told Whoopi has no plans to change back, so producers are struggling to find a way to make it more pleasing to the eye.

10:50 AM PT -- We're told Lindsay was just released from the hospital because her fever broke sufficiently, but her joint pain is still intense. Doctors referred her to a specialist.

We're also told Dina Lohan is on her way to London.

Lindsay Lohan is in a London hospital with a rare, incurable virus she apparently picked up in Bora Bora ... TMZ has learned.

We're told Lindsay contracted chikungunya, a virus transmitted by mosquito bites that causes fever, joint pain and fatigue.

Lindsay vacationed in Bora Bora over the holidays and then flew to L.A. to shoot an Esurance commercial ... we believe for the Super Bowl.

Lindsay began complaining of the illness just before New Year's. She then went back to London where she became so ill she couldn't walk.

Lindsay is currently in King Edward VII's Hospital ... the same place the Queen gets treated. They're keeping Lindsay because she has a high fever and the joint pain is unbearable.

As for her prognosis ... we're told the joint pain could last for months.

BJ Penn slammed his head against a solid wooden bar 3 times in anger ... before the ex-UFC champ went outside and got into a fight this weekend ... bar sources tell TMZ Sports.

We're told ... BJ was a "perfect gentleman" when he arrived to the South Shore Tiki Lounge in Maui on Saturday night -- but became agitated and at one point began banging his head into the bar.

"He was definitely aggravated at something," one source tells us who also says Penn did NOT appear to be drunk.

"He only had one drink. At first he was okay, then he was hitting his head on the bar .... and 20 minutes later he was outside fighting."

As we previously reported, Penn allegedly attacked one of his friends in the parking lot -- and reportedly injured the guy so bad, he was hospitalized for a broken eye socket.

Stephen Belafonte has done what Mel B did last month ... landed in the hospital ... TMZ has learned.

Stephen showed up at the Cedars-Sinai ER Tuesday AM ... complaining of back pain. We're told he says he doesn't know how he hurt it ... but it's gotten bad enough that he needs medical help.

Mel B took him to the hospital and is by his side.

Peter Mayhew, a.k.a. Chewbacca, has been hospitalized for pneumonia.

The guy who's played Chewie since the '70s cancelled an appearance over the weekend at a comic shop in Texas ... and "Star Wars" geeks went into panic mode when they heard he fell ill.

Peter's wife had good news for his fans, writing on Reddit, "He will make a full recovery, and hope to be discharged within a few days. Doctors are incredibly happy with his improvements."

The 70-year-old is set to reprise his role in the upcoming trilogy ... including "Star Wars: The Force Awakens."

11:30 AM PT -- Tiger's agent Mark Steinberg says the tooth was knocked out at the skiing event ... claiming a photog accidentally smacked the golfer in the face with a camera after the race.

"During a crush of photographers at the awards podium at the World Cup event in Italy, a media member with a shoulder-mounted video camera pushed and surged toward the stage, turned and hit Tiger Woods in the mouth."

He adds, "Woods' tooth was knocked out by the incident."

Hole in one ... mouth.

Tiger Woods proved the old adage ... old wounds never heal ... when he flashed a toothless grin after his GF Lindsey Vonn won a World Cup alpine skiing race in Italy this weekend.

Tiger was undercover at the race ... courtesy of a mask featuring a skull ... which had a full set of chompers.

But when he moved in to kiss the victorious Lindsey, the mask went down and Tiger showed what appeared to be the remnants of what became one of the most famous domestic disputes ever.

So, how'd he lose the tooth? Rewind to that Thanksgiving night when Elin Nordegren reportedly threw a phone at him, destroying at least one of his pearly whites.

"Real Housewives of Beverly Hills" star Yolanda Foster says her Lyme Disease has gotten so bad, she can no longer read or write.

In a blog for Bravo, Foster (who has had Lyme Disease for three years) says she relapsed last month and has had a hard time recovering.

She writes, "I have lost the ability to read, write, or even watch TV, because I can't process information or any stimulation for that matter. It feels like someone came in and confiscated my brain and tied my hands behind my back to just watch and see life go by without me participating in it."

Foster describes herself as being "of no use to anyone or anything," but says she will continue to fight and is thankful for the support from her family and friends.

Early Monday morning, Yolanda posted a photo from a hospital in Singapore, writing, "I guess God gives his hardest battles to the strongest soldiers."

Leon Spinks has ALMOST conquered the chicken bone that nearly killed him -- because he's finally out of the hospital and recovering at a rehab center ... TMZ Sports has learned.

We broke the story ... the boxing legend underwent several major surgeries after a piece of a chicken bone (which he ingested during a chicken wing eating session) got stuck in his intestine and wreaked havoc on his body.

But good news!!! We spoke with Leon's wife Brenda who tells us the former heavyweight was finally released from the hospital around Christmas ... and has been in a rehab center ever since working on getting his strength back.

"He's doing much better but it's a slow process," Brenda says. "He was in bed for two months. It takes a lot out of you."

We're told the doctors let Leon spend the holidays at home so he could be with his family and friends ... and then moved him back into the rehab center where he's been recovering ever since.

So when will Leon finally get to go home for good?

Devon Still's ex is waving the white flag in their fight over child support -- telling TMZ Sports the two sides have reached a financial agreement that's in the best interest for their daughter Leah.

The Cincinnati Bengals player -- whose 4-year-old daughter is ferociously battling cancer -- had been at odds with Leah's mother ... who claimed Devon wasn't living up to his end of a "voluntary" child support arrangement they struck over the summer.

Leah's mother, Channing Smythe, claimed she needed the cash for living expenses ... saying she couldn't work because caring for Leah is a full-time job in itself.

But now, Channing says the two sides were able to work out their money issues -- telling us, "He is helping me out financially" ... noting that Devon is still playing a huge role in parenting Leah.

"I am very happy that we are working together for Leah."

One of the "Mob Wives" got whacked -- Ramona Rizzo was hit by a car while walking in NYC, and the perp fled the scene.

TMZ has learned NYPD is investigating the incident -- Rizzo says she was crossing Fifth Ave and 55th St. on Tuesday night when she was hit. Her rep says RR suffered bruises and internal bleeding.

An ambulance took her to a hospital ... where she got treatment for a couple of days.

Ralphie May was a mile high during his concert in Colorado Thursday night ... so wasted the cops had to step in.

The concert started out well ... the opening acts were great. But things went South when Ralphie tried to take the stage ... he was so slow it took forever.

When he got the mic things got worse. Audience members tell us he couldn't string a sentence together, couldn't finish jokes, couldn't remember what he had just said.

Audience members started yelling at him to start telling jokes, then demanded refunds. Some die-hard supporters yelled back at the hecklers and it went all Cosby.

Ralphie finished the show, but by then cops had come to maintain order.

Ralphie's camp tells TMZ ... "He was like a kid in a candy store, being in Colorado for the first time since pot became legal." They add, "We believe in conspiracy theories, we think Cosby snuck in and drugged him. We're pressing charges because we're within the statute of limitations."
