Flo Rida's son Zohar was rushed to the ICU after falling out of a window 5 stories up, and the kid's mother is taking the building owners to court over it ... TMZ has learned.

According to legal docs, obtained by TMZ, the 6-year-old's mom Alexis Adams filed a lawsuit Monday against various entities she claims are responsible for her son's fall from the Jersey City, NJ building. The defendants include a construction company and a window installation company.

Flo Rida is not a plaintiff in the suit ... and DailyMail reported, back in 2018, that Flo hadn't even met Zohar. The current status of the relationship is unclear.

In docs, Alexis says the incident happened at the beginning of March ... and she claims the defendants' negligence led to Zohar's fall from the 5th-story window to the concrete pavement below, leaving him with serious injuries. The little boy reportedly suffered a lacerated liver, collapsed lungs, fractures to his feet and pelvis, and internal bleeding.

She points the finger specifically at "incorrect sized guards" on the windows, and told News12 New Jersey she was completely heartbroken by what happened, adding ... "This feels like a nightmare."

Alexis is now seeking damages, including money to cover Zohar's medical bills. We've reached out to Flo Rida for comment ... so far, no word back.

Raquel Leviss is still spending quality time with Tom Sandoval, and they're cozy enough to be hanging together in the very home where Tom lives with Ariana Madix for years.

The "Vanderpump Rules" villainess -- to most fans -- was outside Tom and Ariana's place tossing a bag into the trunk of her car Wednesday around 4 PM.

Here's the thing ... Raquel was wearing some very comfy clothes and slides as she made her exit.

As for Tom's location, well he left his home about half an hour after Raquel ... on his way to catch a flight outta town.

Sandoval made his way to LAX around 7 PM, catching a Southwest flight to his hometown of St. Louis. He was flying solo, so apparently, he's not ready to bring Raquel home to meet the fam.

Photogs brought up the elephant in the room at the airport -- as well as a couple more Qs, but the guy wasn't in the mood to chat, instead makin' a beeline to check his bag.

However, the fact he and Raquel are close enough to kick it where he lived with his ex is pretty telling about how close they remain, now that the 'Scandoval' dust is settling.

As we reported, Raquel was a no-show in court Wednesday for the hearing over her restraining order against Scheana Shay. She'd alleged Scheana attacked her over the affair ... but since she skipped court, the restraining order was dismissed.

Now we know what, or who, was more important than court for Raquel.

Eva Marcille is throwing in the towel on her marriage with Michael Sterling ... 'cause she wants a divorce.

The 'Real Housewives of Atlanta' star just filed docs in Georgia, obtained by TMZ, asking the judge to officially end her marriage. She also makes clear that she and Michael are currently separated.

In the paperwork, Eva doesn't cite or specify any given reason for the divorce -- other than to say the marriage is irretrievably broken and there's no chance of reconciliation, which is essentially irreconcilable differences. She does, however, lay out what she'd like to see when it comes to their three minor children, who are all younger than 10 years old ... EM is asking for legal and primary physical custody.

Eva says she should be the parent who has the final say on all decisions related to their kids, as she feels she's the fit and proper caregiver.

Another thing she wants from Mike going forward ... child support. Eva says he oughta contribute toward the kids' medical, dental, and hospitalization expenses.

When it comes to assets, Eva says she wants an equitable split of everything they've acquired in the marriage and any debts they've racked up.

Quavo just announced he's been cooking a new "bun" in the oven and is ready to pull the finished product from the studio!!!

On Monday, Quavo posted a pic of himself in a fur coat, puffing on something ... and, in the post, revealed he had something called "Honey Bun" dropping this Friday. He dedicated the rollout to his late nephew Takeoff.

Nick Lachey has been ordered to attend anger management and Alcoholics Anonymous meetings after going off on a paparazzo in Los Angeles ... TMZ has confirmed.

Nick flew off the handle last year after spotting a woman who was filming his wife and himself after a night out a Mastro's in Bev Hills. Nick was enraged and caught on camera reaching for the woman's phone.

The L.A. County D.A. tells TMZ ... Lachey was never charged with a crime ... instead, the D.A. has a diversion program where someone can avoid prosecution ... in Nick's case, he won't be charged with assault and battery if he attends AA and anger management.

The woman, Jody Santos, spoke to Daily Mail about the incident, saying, "He's had plenty of time to apologize, but instead he just figured I would go away. What he did was wrong. Attempting to bust his fist through my car window when all I was doing was taking photographs of him. He should have been thanking me for the free publicity. I mean, really. Talk about an oversized ego."

Jimmy Fallon left the Big Apple for St. Patrick's Day and ventured upstate for some small town pub partying ... hitting up a local watering hole, and jamming out with the band!

The 'Tonight Show' host found his way into Iron Smoke Distillery Friday in Fairport, NY -- just outside of Rochester. He actually knows the owner of the bar across the street -- Mulconry's Irish Pub -- and apparently heard the musicians from 1916 rocking out ... feeling intrigued.

We're told Jimmy popped in toward the end of 1916's set and asked if he could play with them before they headed out. Of course, they obliged -- sticking around for a few more songs. As many people know, JF's a bit of a music man himself ... and a natural showman.

That came in handy for the three tunes they performed -- including The Doors' "Gloria," The Georgia Satellites' "Keep Your Hands to Yourself," U2's "Desire" and others. Jimmy took charge on vocals and on lead guitar ... and all the other band members backed him up.

After hitting the stage, we're told Jimmy hung around a bit and took photos with patrons ... capping off a great time had by all. Seems he wanted his holiday to be a little more low-key and homegrown ... and he certainly got that with the Iron Smoke crowd that night.

We'll see if he has more to dish about his night out come Monday ... when he'll be back on late night duty, with his own band on hand. This is just a good reminder -- dude could probably jump in himself at a moment's notice and hold his own.

The 2 rap stars were beaming from ear to ear as they arrived Monday night at Mr. Chow for a romantic dinner date -- and even with a photog there, they weren't shy about showing their affection.

French threw his arm around Rubi shortly after they got out of his ride, and they strolled into the restaurant looking totally comfortable being a new hip hop power couple.

A source close to the couple tells us Rubi is 100 percent French's "new gf." It's unclear when exactly they started hanging out, but it's clear they're past that initial "getting to know you" phase.

Rubi's a former XXL Freshman with several releases under her belt and more than 4 million IG followers.

"Vanderpump Rules" star Scheana Shay isn't holding back on Raquel Leviss after she accused her of punching and injuring her ... calling Leviss a "liar and a cheat who has betrayed everyone close to her."

Scheana's attorney, Neama Rahmani, sent out the blistering statement Thursday, telling us, "This case is a fabrication by a known liar and a cheat who has betrayed everyone close to her. Scheana never punched Rachel [Raquel], period."

TMZ broke the story, Raquel was granted a restraining order against Scheana earlier this week, accusing Scheana of punching her in the head, leading to injuries around her eye.

Rahmani addressed the photos Raquel included in her court filing of the alleged injuries, saying, "The supposed dark markings around her left eye have been there for months."

As for what led to the alleged attack, Raquel says Scheana lashed out after the 'VPR' cast learned Tom Sandoval had cheated on Ariana Madix with Raquel. The judge granted a temporary restraining order until a hearing later this month.

Rahmani continues, "Neither Scheana nor other cast members want anything to do with Rachel going forward. The judge has only heard a one-sided account of what happened, and we look forward to presenting the full and true story at the March 29 hearing."

Country singer Clay Walker is in hot water after getting caught on audio berating his bus driver ... but Clay tells us it went down a year ago, and it's all water under the bridge.

Clay starts by saying he could easily whup the driver's ass. The singer also claims the driver didn't show on time ... and Clay is PISSED.

It's pretty harsh ... and just when it sounds like all is said and done, Clay finds another reason to call him out.

While this is clearly an intense one-sided convo, Clay tells TMZ ... "This happened over a year ago, and it came at the end of a long, tough weekend. The bus driver and I reconciled immediately, and we are friends and stay in touch.”

FYI -- Clay's been in the country game since his debut record in the early '90s ... first topping the charts with his single, "What's It to You."

Tiger Woods' split from his ex-girlfriend is way nastier -- and potentially more expensive for him -- than we thought ... TMZ Sports has learned she's suing him for $30 million, claiming he cooked up a scheme to toss her out of their home.

According to docs we've obtained, Erica Herman and Tiger actually broke up last fall. The then-couple had been living together in his Hobe Sound, FL home for 6 years, during which time Erica says she performed "valuable services" for Tiger in exchange for living there rent-free.

She admits she doesn't have that in writing, but refers to an "oral tenancy agreement," which was supposed to last 5 more years after their split.

About that breakup ... Erica claims Tiger's people convinced her to pack a suitcase last fall for a "short vacation" -- without Tiger -- and drove her to the airport. Once they arrived at the airport, she was told she'd been "locked out" of the house and could not go back.

In the docs, Erica alleges one of Tiger's lawyers even met her at the airport with a proposal to resolve any outstanding issues between Tiger and her. She also claims they confiscated more than $40K in cash she had at the house, and accused her of doing something shady to get the money.

She's suing Tiger's trust -- the titleholder on the house -- for $30 mil because she claims that's the "reasonable rental value" of Tiger's house for the 5 remaining years on their oral agreement.

Tiger's trust filed docs in response to the suit, and said Tiger had broken up with her and asked her to leave the home, which he shares with his 2 children. The trust denies there was ever any oral agreement about her living in the home ... instead, Tiger's team says she was only there as his girlfriend.

Basically, once the relationship ended ... so did her right to live there.

"Vanderpump Rules" star Tom Sandoval has a lot on his plate while deep in a cheating scandal with his costar, Raquel Leviss ... and it appears he's talkin' it all out with a shrink.

Tom was spotted heading into a building in L.A. Tuesday -- one that's home to many therapy and psychotherapy offices, we're told.

A source with direct knowledge tells us he was with a therapist for about 2 hours, although this isn't a new thing ... he was in therapy long before this scandal.

As you're well aware, Tom was caught cheating on his longtime GF Ariana Madix with Raquel ... our sources told us Tom and Ariana have no plans to get back together, and 'VR' is diving deep into the messy breakup as part of the latest season.

Siegfried & Roy's former Las Vegas estate is on the market ... the same palace where they famously lived alongside their lions and tigers.

The 8-acre estate is known as the 'Jungle Palace' ... and it's where the late Roy Horn raised the jungle cats for the magic act's Sin City show. Roy even slept in the same bed as the lions and tigers and swam with them in one of the home's 3 pools.

Listed for $3 million, the compound features an 8,750-square-foot main house, plus 3 guest houses, 2 detached studios, 6 electric gates, 3 pools and a jacuzzi.

Built way back in 1954, Siegfried & Roy bought the Moroccan-themed compound in 1982 ... and it's where Roy lived until his May 2020 death from COVID complications.

The property reportedly changed hands a year ago, with the current owner snapping the place up for $1.87 million.

Ben Affleck and Jennifer Lopez pulled the plug on a mansion they were trying to buy -- but for good reason -- 'cause the new one they want is way nicer ... and twice as expensive.

Real estate sources tell TMZ ... the married couple is currently in escrow for a massive property in the Pacific Palisades, the same neighborhood where they were just in escrow for another house ... only to reverse course at the last minute, as we first reported Sunday.

This new crib is quite the palace ... whereas the mansion they just bailed on was worth about $34 million, this one they now have their sights on is valued at a whopping $64M!

The price tag is certainly high, but it might be worth it when you consider everything it comes with -- including 8 bedrooms, 11 baths and 16,000 sq. feet of a fortress on a 1.13-acre compound. Yeah ... it's humongous -- and that's not including everything else inside.

Mind you, this is the property that Bennifer recently visited in person ... and our sources tell us they've been swinging by frequently to eyeball changes they might wanna make once the sale goes through. They do this often, BTW -- even at the other PP house they nixed.

Oh, and if you're wondering how Ben/Jen can afford such a pricey pad -- beyond just being filthy rich -- we have answers on that as well. Our sources say Jen's famed Bel-Air estate, which has been on the market for a minute, is also now in escrow with a potential buyer for $39M.

Add on the fact Ben recently sold his own Pacific Palisades mansion for around $30 mil, and they've got plenty of money for the new crib.

Tom Sandoval and Ariana Madix seemed to be very much a couple just days before TMZ broke the story of their split ... and we've now learned more details about what specifically led to the couple's sudden end.

Sources close to the former pair tell us it was a series of text messages Ariana discovered Wednesday night between Tom and Raquel Leviss that caused her to believe he was cheating. We're told the messages and video were "sexual in nature" ... leading to a confrontation between Ariana and Tom.

Our sources tell us from there, Ariana told friends she found out Tom and Raquel had actually been seeing each other secretly for months -- as far back as the summer.

We're told there was growing suspicion between the former couple's friends that something may have been going on between Sandoval and Raquel ... but it was totally a surprise to Ariana.

As we told you, Ariana was spotted supporting Tom Wednesday night while he performed with his band and debuted a new single. In fact, eyewitnesses at the show say they were kissing and having a good time together -- very much still a couple.

Apparently, Tom has a different version ... a source close to Tom told us things had been unraveling between him and Ariana for quite some time ... but gotta say, that wasn't evidenced Wednesday before she'd discovered the text messages.

Tom and Ariana got together back in 2013 when she joined "Vanderpump Rules" as a full-time cast member ... while they never got married, they were certainly one of the anchor couples on the reality show for years.
