Daisy Coleman's mother was gravely concerned about her daughter's state of mind shortly before her suicide, but tragically ... even a wellness check couldn't prevent it.

TMZ broke the story ... Daisy -- one of the teenage girls featured in the Netflix documentary "Audrie & Daisy" about multiple rape cases -- took her own life Tuesday night. We've since learned that she died in Lakewood, Colorado.

John Romero, spokesperson for the Lakewood PD, tells us Daisy was at an apartment around 4 PM that day when officers were called to make a wellness check for a "possible suicidal party" at the residence.

As we told you, the person who called this in was Daisy's mom, Melinda. Romero says the police department responded by sending paramedics and agents trained in crisis intervention.

We're told Daisy talked to the agents for over an hour, but she never said anything or gave any indication she wanted to harm herself ... so she was medically cleared, and there was no legal reason to hold her for mental health issues.

Sadly, we’re told police received another call around 8:30 PM from a female friend of Daisy's, reporting that she had shot herself inside the same apartment. Cops say Daisy used a pistol to fire the fatal shot.

Daisy was one of the subjects of the 2016 Netflix doc, which followed her and members of her family as they dealt with the trauma of her alleged sexual assault in January 2012 when she was just 14 years old ... as well as the hostile reaction of her Missouri community.

Daisy was 23.

Jorge Nava is finally following through with his promise to end his "90 Day Fiance" marriage to Anfisa Arkhipchenko ... he's officially filed for divorce.

Jorge filed this week in Arizona, only a few days after the former couple marked their 3-year wedding anniversary.

According to the docs, obtained by TMZ, Jorge lists Nov. 19 as the date of separation. He cites the usual irreconcilable differences as the reason for the split.

There doesn't appear to be major hurdles ... Jorge and Anfisa did not have any children together and in the docs, he says they don't have any property or debts together.

We broke the story ... Jorge vowed back in March to leave his Russian bride after finishing his prison stint for drug charges. He claimed Anfisa abandoned him while he was behind bars and ran off to be with another man.

Jorge and Anfisa were a big hit during season 4 of the show, and on Aug. 1, 2017, they tied the knot.

As we first reported ... Jorge shed a whopping 128 pounds while doing hard time, and now it seems he's cashing in on his new and improved look -- posting a lot of loving pics with a new woman.

When it comes to love, Kamal "Chance" Givens is living up to his name ... 'cause he's returning to TV for a dating show, and this time ... he's the star.

Chance -- best known for pursuing Tiffany "New York" Pollard on VH1's "I Love New York" -- is back in the game with his own dating competition reality series dubbed "One Mo' Chance."

And, get this ... Ray J and Princess Love -- despite their break-up -- are executive producers.

The new show premieres this fall on the video-on-demand network, Zeus ... and it gives Chance another opportunity at love and to share how his life has changed since finishing as the runner-up on "I Love New York."

We're told the show is still filming "bubble style" where contestants have been tested and quarantined together.

He'll touch on a bunch of subjects ... from the relationship he has with the mother of his kids to his brother Ahmad "Real" Givens' death from colon cancer in 2015.

You might recall Chance and Real had their own reality dating show, "Real Chance of Love" -- which ran for 2 seasons in 2008 and 2009.

GOOD NEWS -- Daniel Cormier just revealed his wife is 7.5 months pregnant!!!

And, to keep her safe, DC says he's been training for his rematch with Stipe Miocic in a "bubble" environment to prevent any possible COVID-19 exposure.

FYI, Cormier will step into the Octagon for his rubber match with Stipe at UFC 252 on August 15 to fight for the heavyweight title.

41-year-old Cormier insists this will be his farewell fight to cap off his Hall of Fame career ... and he's been training like a maniac to get ready.

But, DC's not naive about the pandemic and realizes that his training has required him to interact with a bunch of people from the outside, raising his risk of catching coronavirus.

So, he's been living at a 2nd home that he owns to limit the interactions with his own family ... because he doesn't want to get anyone sick!

"Yeah, I got a pregnant wife who's 7.5 months pregnant. Two kids, a 9-year-old, an 8-year-old, and I'm not even living at home because I'm having to prepare for this fight -- but that also shows the commitment from the family to understand what I'm trying to do here is so special."

In other words, Cormier says everyone is making a sacrifice so he can put on the best performance possible at UFC 252 ... because going out as champion is extremely important to him.

"I think that all the things I've done at heavyweight and at light heavyweight makes me one of the greatest fighters of all time, period! But, if I become a 2-time heavyweight champion there's only a few guys that've done that before in their careers. I think that by being the most dominant heavyweight of all time twice would make me the greatest heavyweight UFCs ever seen."

Cormier adds, "Obviously, Stipe's accomplishments and accolades speak for themselves. You know he's won everything. This is a true legacy fight."

"People don't like to talk about legacy all the time, but this a legacy fight in it's purest form."

He's right ... but more importantly, CONGRATS ON THE BABY!!

Before this curly-headed cutie was a teenage heartthrob who was sure to make you scream, he was just another cute kid growing up in Lynchburg, Virginia.

This mullet man gained notoriety in a scary movie franchise but got murdered off in the first film for being the man behind the mask. Even though he was born Bryan Ray Trout, you may be more familiar with his stage name.

Now you can catch him on a popular CW television show loosely based off the "Archie" comics where he plays the role of Cole Sprouse's gang ringleader father.

Here's what happens when doing it for the 'Gram goes wrong -- a tourist in Italy damaged a 200-year-old sculpture while posing for a pic.

The tourist, who hails from Austria, snapped off several toes from a 19th-century plaster model by Italian sculptor Antonio Canova while sitting on the display and trying to mimic the pose for a photo op.

The incident happened last week at the Gypsotheca Antonio Canova museum in northern Italy, and video shows the tourist standing up to discover the damage before getting the hell outta Dodge.

The museum claims the tourist bounced without alerting anyone to the damage, and a museum official noticed the broken toes a few minutes after it happened and sounded the alarm.

Italian police reportedly tracked down the tourist, thanks in part to the pandemic. Museum visitors must sign in and leave personal info for potential contact tracing, and that's how police found him.

The tourist's wife reportedly burst into tears when contacted by cops and admitted her husband was the culprit, and the man has since confessed and apologized for the gaffe.

An Italian court is reportedly weighing whether to press charges, and the president of the Antonio Canova Foundation wants the tourist punished.

The good news ... the artwork's marble version is housed in a gallery in Rome and has, so far, remained undamaged by photo-hungry tourists.

If you've never seen human billiards, you're about to -- a woman knocked off her moped bounced once, and then went straight down a gutter. Worst luck EVER!!!

The strange, but true accident went down in Brazil ... where some jerk (more on him later) driving a car turned right into a moped, sending the female rider flying. She hit the pavement hard, and started rolling.

As if getting hit wasn't bad enough ... her momentum sent her careening directly into a huge storm drain on the roadside!!! Honestly, it's a one in a million chance.

Now, the good news a bunch of Good Samaritans ran to her aid -- one of 'em even jumped down into the sewer -- and they were able to pull her out safely. Amazingly, it looks like she only had some bad scratches on her legs.

The bad news? Watch the video ... while the crowd gathered to rescue the woman, the guy who hit her snuck back to his car and dipped!!!

Someone on President Trump's campaign team is painfully tone-deaf, opting to crank "Live and Let Die" before his Ohio event ... as about 1,000 Americans are killed daily by COVID-19.

Ya probably wouldn't believe it, if there wasn't video, but a reporter covering Trump's speech at a Whirlpool factory posted it -- and Guns N' Roses version of the Paul McCartney classic was, sure enough, blaring on the live feed.

The reporter, Robert Mackey, points out the obviously disturbing nature of the track blaring at a time like this. Not to mention, Ohio's Governor, Mike DeWine, couldn't attend the event because he just came down with COVID-19.

And, of course, the virus is on the rise in a majority of states and killed more than 1,400 Americans just Wednesday.

Bottom line: It's either the worst timing ever or someone's idea of a sick twisted joke. BTW, Mackey says it played twice!

For the record, GnR frontman Axl Rose is no fan of Trump or his handling of the coronavirus, and actually used the song title to mock the President back in May.

The firearms seized in the FBI raid at Jake Paul's house are now in the hands of the Sheriff ... and we're told the guns aren't the reason the feds stormed the place with a warrant.

Law enforcement sources tell TMZ ... there were multiple guns taken from Jake's crib in Calabasas during Wednesday's FBI raid, and they're now being held by the Los Angeles County Sheriff's Department for safekeeping.

It sure seemed weapons may have been what drove the FBI to get a search warrant and, in a massive show of force, raid Paul's L.A. home, as well as his friend's home in Vegas. We do know the raids were somehow connected to looting at a Scottsdale, AZ, mall ... where Jake and his friend were front and center.

As you know ... aerial footage of the scene from ABC 7 showed law enforcement officials walking down Jake's driveway with rifles and perhaps shotguns.

We're told Sheriff's deputies seized the guns once FBI agents entered the home because the house was not secured and deputies couldn't just leave the guns laying around.

Our sources stress the guns were not taken as evidence of any crime, or FBI agents would have taken them themselves.

We broke the story ... Jake had 2 large guns prominently displayed in his bedroom, which were casually displayed in a recent YouTube prank.

A Milwaukee Councilman is slamming Republicans for what he believes is a trick to lure black voters in Wisconsin to cast their ballots for Kanye West and steal votes from Joe Biden ... but he predicts it will backfire.

Cavalier Johnson has been the Alderman for Milwaukee's 2nd district, and he's sounding the alarm for what he calls a "shameful, pitiful" ploy attempting to take advantage of a man struggling with mental illness.

There have been reports Republican operatives are behind Kanye's efforts to get on the Wisconsin ballot. As you know, Wisconsin was a swing state in 2016 and if it's close again, a spoiler like Kanye could make all the difference.

Johnson says ... "It’s hard to imagine how this president and the GOP could top the racism, xenophobia, white supremacy, COVID failures, apologies for Russia, pocketing millions in taxpayer dollars with visits to Trump-branded golf courses, etc.," but he accuses them of doing exactly that in Wisconsin.

Despite his outrage and disgust, the councilman doesn't think the plan will work, saying when the word gets out it will further unite the black community in support of Biden.

As we reported ... Kanye has enlisted the help of a third-party organization in order to acquire enough signatures needed to get him on the presidential ballot, especially in major swing states like Wisconsin and Ohio.

Justin Hartley cold-heartedly fired off a text to Chrishell Stause about filing for divorce ... so claims the "Selling Sunset" star.

Chrishell broke down telling co-star Mary Fitzgerald about exactly how she found out the "This Is Us" star wanted out of their marriage. It's pretty heartbreaking ... Chrishell choked back tears before revealing, "I found out because he texted me that we were filed [for divorce]."

The actress/real estate agent says at first she thought Justin was joking but pretty quickly reality set in ... adding that "45 minutes later the world knew." TMZ broke the story ... Justin filed for divorce in November 2019 after 2 years of marriage. She later strongly hinted she was blindsided by the news ... and now we know why.

Chrishell says she spoke to Justin after he dropped the bombshell on her but that was the end of their communication. She was clearly distraught ... saying even though they were only married for 2 years they were together for 6 years. She said if this is what Justin really wanted "there are better ways to go about it."

The Netflix hit "Selling Sunset" premieres season 3 beginning Friday ... and the season's storyline largely centers around the sudden end of Chrishell and Justin's marriage.

That's playing out pretty publicly -- Justin's moved on and has been photographed getting cozy with a new GF.

Dutch cyclist Fabio Jakobsen nearly lost his life in a horrific crash in a race in Poland this week ... and he's now in a medically-induced coma recovering from serious injuries.

Jakobsen was jockeying for first place at the end of the Tour of Poland on Wednesday ... when he appeared to be cut off by fellow cyclist Dylan Groenewegen.

What happened next was terrifying ... Jakobsen flipped over some signage lining the raceway and then crashed face-first into a solid barrier.

Jakobsen suffered major injuries in the wreck ... his cycling team announced he had to undergo facial surgery and then be put into a coma.

But, thankfully, team officials say Jakobsen avoided serious spine/neck ailments.

"Fabio had facial surgery during the night," the cycling team said in a statement Thursday. "His situation is stable at the moment and later today the doctors will try to wake Fabio up."

Groenewegen was heavily criticized for his role in the crash ... he was DQ'ed from the race despite finishing first -- and was blasted by Jakobsen's team's GM, who called Groenewegen's end-of-race maneuver "criminal."

For his part, Groenewegen apologized on Twitter, writing, "I hate what happened yesterday. I can't find the words to describe how sorry I am for Fabio and others who have been dropped or hit."

"At the moment, the health of Fabio is the most important thing. I think about him constantly."

Meanwhile, U.S. cycling legend Lance Armstrong sent his best wishes to Jakobsen, saying, "Thoughts and prayers go out to Fabio Jakobsen. Hang in there buddy."

Country music star Travis Tritt is not rotating around the Earth, but his music is getting some serious space spins ... because NASA astronauts made their own fun video to his hit song!!!

NASA commander Chris Cassidy is the astronaut in the center of this awesome homage to Travis' hit, "It's a Great Day To Be Alive."

Chris and the other folks onboard the International Space Station are doing alright 250 miles above Earth ... they got rice cooking in the microwave and some homemade soup ... so Chris says they're feeling pretty good and that's the truth.

As for the musical choice, Chris says NASA's Expedition 63 crew "is very much aware of the hard times which exist in the neighborhoods of the world rotating below," and he wants folks to remember ... "Cherish the people you love."

Chris ends with some words to live by, whether you're firmly planted on terra firma ... "Even amid the uncertainty and difficulties of battling through these challenging times, we encourage everyone to be the best 'crewmates' and take care of each other."

The former "Alias" stars hit up Malibu beach Wednesday, along with a little girl who appears to be Bradley's daughter, Lea De Seine Shayk Cooper.

Bradley and Jen have something in common ... they've both split from their partners ... obviously Jen more recently than Bradley.

The 2 have been seen together over the years, but now that they're both available ... who knows? But, if you can glean anything from a few snapshots ... sure looks flirty.

There were rumors during their "Alias" days that costars Bradley and Jen may have been a thing, but it never materialized ... at least not publicly.

Who knows whether this is something, but when people break up they often gravitate to people they know, and lots of times that develops into something. If you didn't know that ... it's actually a thing.
